Anything more than a flash tattoo or a straightforrward design is best to be worked out thoroughly in advance.
I mean how many times have you heard the story of someone explaining in detail what they wanted and the artist kinda did something else. Not what they wanted.
Lost in translation is a common thing that can easily be avoided by simply going through a design consultation.
This is a thorough and detailed consult taking pics of where you're wanting it and overlaying and ajusting elements that bring your story together. They all need to be scaled and arranged in a way that reads well on your body. This avoids any nasty surprises and potential errors.
Depending on the complexity and the scale of the project this might take anywhere from half an hour to around 2 hours. This then provides us with a plan to work from, a blueprint to schedule and work from.
Having this in place means each session we know upfront what we'll be working on and approx how long it will take.
